Use this form to reserve domestic or international travel (including airfare, car rental, and/or hotel) using the University Travel Advisors. If you need to make a change to an existing reservation, please contact Christopherson Business Travel.
If you only need to book a Car and/or Hotel, please use this form.
A University Travel Advisor will respond with on-hold travel reservations using the information provided on this form. Travel agency hours are M-F, 8-4. Forms submitted outside of office hours will not be processed until the following business day.
An Approved Concur Request ID is required to submit this form. This form must be submitted, or the travel advisor will not be aware their assistance is needed.
University Travel Advisors can reserve:
- Employee Travel + Additional Traveler(s)
- Guest Travel
- Student Travel
*Note: Do not use this form for group travel. Please submit the Group Travel form for travel agency assistance.
ON HOLD Reservation:
- This form will result in an “ON HOLD” reservation that will be emailed to the traveler/travel arranger to approve ticketing.
- ON HOLD airfare is only held for 24 hours and the price is not guaranteed until the ticket is issued.
- ON HOLD trips with airfare require completion of the Confirm a Trip form. Trips with no airfare do not require this step.
- Departments are encouraged to confirm ON HOLD reservations containing airfare as soon as possible to keep airfare costs low and avoid airline penalty fees associated with lengthy holds.

Due to increased airport security, you may be turned away at the gate if the name on your identification does not match the name on your ticket.
